In-person Evaluation + Sessions
Initial Evaluation: 60-90 minutes, $250.
Follow-up Treatments: 45-60 minutes $200.

*Balance Back to Life PT is an out of network provider. We accept Zelle, Venmo, HSA, check, credit/debit card. We can supply you with a Super Bill after your PT sessions for you to submit for reimbursement. We are not a Medicare provider.*

We do not take any health insurance for our services.

The process is very simple. Balance Back to Life provides you with an invoice/superbill with all the details needed for your insurance company.
Medicaid and Medicare plans do not provide reimbursement. -
With our superbill, you have everything you need to submit for health insurance reimbursement.
You can do this at your own convenience and most insurance companies require it within the year of the treatment so there is generally no rush.

We can provide a full 60+ minute session with a complete patient focus.
It avoids co-pays (which can be almost as much as our hourly rate at times), limitations to therapy location and number of sessions.

Cash, personal check, Venmo and Zelle are all accepted.
